YFS’ Intensive Family Support (IFS) program is looking for an Intake and Support worker to process, assess and triage referrals for families with complex needs. You will also provide interim weekly telephone support until a case manager is assigned.
We see this role as the heartbeat of the team. You will be responsible for assessing risk for families in crisis, linking them to supports and ensuring they feel heard and respected. You will also work closely with the Program Manager and the Practitioners, as a central touchpoint.
The ideal person for this role will be an experienced case manager with a strong understanding of trauma-informed practice and child development.
As part of the YFS team you will liaise across YFS programs, and external agencies to source immediate supports and de-escalate crisis situations.
Your highly tuned risk assessment skills will help to ensure the safety of children and best-practice supports from team members.
