We are looking for an experienced casual Home Support Assessor to undertake face to face assessments using the mandated National Screening and Assessment Form (NSAF), create Support Plans, and match and refer clients for a Comprehensive Assessment, service delivery or linking service support such as case management, assistance with housing, or non-aged care support.

This role reports directly to the Manager, Regional Assessment Services. Please note this is a community based role, which requires you to have a valid drivers’ license and access to a vehicle.

About You:

• Demonstrated ability to work respectfully with older people and their families
• Understanding of normal ageing processes and complex co-morbidities
• Ability to undertake a strengths-based assessment and facilitate goal focussed care plans
• Demonstrated ability to undertake culturally appropriate assessments
• Effective time-management ability
• Highly developed communication skills – verbal and written
• Commitment to promote and apply a wellness and reablement philosophy with older people
• Demonstrated understanding of the interaction of social, physical, emotional, familial and environmental factors at an individual level and the ability to work with an older person to develop a truly personal goal-oriented support plan
• Understanding of the formal and informal supports available to assist older people live safely at home in Sydney
