The Care Navigation service model enables a focus to support people, their families, and carers to navigate the health and social care systems to access the services they require, when they require them. Proveda aims to ensure that everyone gets to live the life they want.
In this role you will lead the delivery of a wide range of customer focused, customer-led and co-ordinated community care services in accordance with the relevant practice standards across NDIS, Aged Care & Mental Health services.
The Manager, Service Delivery will also play a critical role in helping shape the Proveda of the future in line with an ambitious strategic plan. This involves continuing the evolution and redesign of service delivery, to meet the changing needs of customers, improve quality and adapt to regulatory reforms. This involves harnessing exceptional practice and people leadership, business acumen, and a passion for facilitating change well.
