About you

Fantastic opportunity for a passionate, skilled and experienced Case Manager to join CatholicCare Foster Care services in Sydney or on the Central Coast. The role is a full-time, permanent position.

As a Case Manager, you will work with young people and their care teams to deliver case management across a range of therapeutic services.

As a Case Manager, you may work across any of our locations on the Central Coast and in Northern Sydney. A requirement to travel to Sydney’s Northern Suburbs when required, will be highly regarded.

Flexibility to work some additional hours, including at weekends, and to undertake on-call duties is required for this role.

Key responsibilities include:

  • Work with and support foster and kinship carers to provide holistic and therapeutic day to day care of children and young people.
  • Case planning and holistic assessment and support of families, children and young people to promote and support their safety, well-being and permanency needs.
  • Working closely with young people and their families, as well as internal and external stakeholders, to develop pathways for the young people to transition through care and ultimately leave care through an appropriate permanency pathway.
  • Timely completion of administration tasks and case notes.
  • Any other duties as requested by the Manager.

To be successful in this position, you will have:

  • A Degree qualification in a relevant field and ideally have additional training or qualifications in developmental trauma, child protection, client intervention and case management.
  • Demonstrated case work experience working with children and young people who have experienced complex trauma.
  • Experience working with families who have been in contact with the child protection system.
  • Exceptional written and oral communication with the ability to build relationships
  • Full NSW driver’s licence and own comprehensively insured vehicle.

Introducing Permanency Support Program (PSP):

PSP provides an alternative to long term Foster and Kinship placements, the program focuses on permanency for young people and their exit from the system through pathways such as restoration, guardianship and adoption.
