The Koorie Connect program offers information, advice, and casework for clients with complex needs in Melbourne's Northern metropolitan area. The Caseworker provides phone, office visits, and outreach support, aiming to deliver holistic, wrap-around services. This includes assessing client goals, developing a plan, and connecting them with the right services.
Provide intensive casework, including assessment, goal setting, and advocacy, to support individuals and families in achieving their goals. Build relationships with service providers to improve access for Aboriginal people and maintain accurate client data on CSnet. Follow VACCA's policies and participate in meetings, team support, professional development, and other duties as directed.
