Are you a self-starter with a passion to support expecting and parenting young parents to access services and supports that will make a difference?

This is an exciting new role where you are able to follow a family's journey from pregnancy to development and link them into the key supports, they will need as a young family.

  • The Community Connector is a hybrid role that works across two programs with the goal of creating increased synergy across these and building mothers’ access to informal and formal supports.
  • This position provides a chance to build on current and create new and effective ways to link across health and community systems and services to get the right supports for pregnant and parenting young parents at the right time.
  • You will be part of an innovative model of midwifery care based in the YFS Maternal and Child Health Hub and part of the wider Logan Hub network.
  • Support child development and parents learning and connecting through a peer space.

About the job

  • Role location: Slacks Creek
  • Role type: Full time - 76hrs per fortnight, 6-month contract
  • Pay range: $43.08 to $44.20 an hour based on experience, with salary packaging exempt from Fringe Benefits Tax available

In this role you will:

  • Working alongside a team of Metro South Midwives, you will engage with pregnant mothers to support their journey toward a healthy and safe delivery.
  • Mothers accessing the YFS Maternal and Child Health Hub have complex psychosocial needs. Your understanding of trauma informed practice will underpin the way you engage with them to link them with key services and supports.
  • You will liaise across multiple YFS and community programs to keep current with changes and opportunities for mothers.
  • Our goal is to improve pregnancy outcomes and set children up for a positive future.
  • This role will also support the Step-by-Step team to facilitate peer spaces for both children and parents to build their community connections and life skills.
