Based at a confidential location in Western Sydney, the Therapeutic Case Manager provides individual, family and group support planning to women, young people and children who are clients of our DFV Transitional Housing Service. You will provide practical support and interventions to increase client’s safety and wellbeing post refuge through the lens of a multi-level practice framework in collaboration with the team.

Your role will include:

  • Determine appropriate interventions that apply theoretical knowledge of domestic and family violence
  • Conduct risk assessments and respond to identified risk through a collaborative team approach
  • Provide practical support to women and children and advocate on their behalf
  • Support the children’s wellbeing and development, and parents’ skills & confidence
  • Undertake individual therapeutic support planning for children and women to meet their goals, hopes and aspirations
  • Network to develop collaborative relationships with stakeholders to successfully link families to support services during and post exit
  • Ensure quality services that enhance the lives of the families that we support
  • Ensure support prioritises the safety and wellbeing of children and that every phase of case management the focus is on the best interests of the child.

what we are looking for

We are looking for a flexible and adaptable individual who will work collaboratively as part of a team to achieve best practice outcomes for woman and families.

You will bring:

  • passion, commitment and belief in young people
  • bachelor level qualification in a Human Services discipline and a minimum of three (3) years’ case work experience with women and children who have experienced domestic and family violence; or a Diploma level qualification and a minimum of five (5) years’ relevant experience
  • ability to work within a clear client centred, trauma informed strengths-based framework
  • experience in providing therapeutic support for women and children
  • experience in planning, implementing and facilitating individual supports and groups for women and children who have experienced domestic/family violence
